The best ones are always ones that the rest of the film suitably built up to rather than it being a stand alone scene. Like these:

    The Rock

    Rocky

    Rocky Balboa

Absolutely gut wrenching... The Green Mile

The end sequence scene in Men in Black 3 which puts a lovely cap on the trilogy

Massively indulgent but it did genuinely mark the end of an era within the show and after such a long build up it hit its mark... Doctor Who - Tenant and Davies final scene

Although when it wants to modern day Doctor Who can do a good episode ending:

Stop before the ill advised Season 9 and the true ending to Scrubs fits well

And then probably the most heartbreaking episode ever. Futurama... waiting for Fry to come home...

The clever bit about that was the whole thing about...

...he knew that rhythm would be the cause of his death.

Then, the whole season running up to it, it shows up in loads of places where he's in danger, and finally, once he's won, once he's managed to save everyone...

        <tap tap tap tap>

        It was a great twist.

              Totally

that shot of him on the floor when it looks for a second like he's beaten it and then you hear the knocks and it's poor Wilfred. Every bit of it from there on played out perfectly

There's no vid but another came to mind, the ending to Series 3 of Being Human:

Three series of storyline built up to a low key, emotional face off as one of the main characters pleads with the other to kill them. No action, no visual effects, just three years of build up and the actors ability to sell that moment. It was the peak of the shows entire run and it should have found a way to end there as it literally never recovered and the two series that followed saw the cast desert and the quality tail off as the outline of the story was clearly not supposed to see that character leave when did originally. Brilliant moment though

The last act of The Raid 2. Such a satisfying payoff to that which has come before it, I'm not sure it will ever be topped.

Films - Most have been mentioned already

                    Babylon 5

end of season 3 with Sheridan's kamikaze on the shadows home home preventing B5 destruction. and then most of Season 4 build up to 3-way war ending in telling the 1st ones to get lost. And while not much of a climax the shutting off the lights to B5 at the very end.

Could go on forever about tons of anime, so i'll just keep it to one most might have seen.

Cowboy Bebop - one the best personal grudges,

Spike a slave to his revenge sends his crew away before the final showdown with Vicious, followed by BANG! and Blue playing in over the credits. You're going to carry that weight.
